Cobble Hill Health Center is hiring Licensed Practical Nurses (LPNs) to join our compassionate care team in New York. We offer FT/PT shifts: 3pm-11pm and 11pm-7am, competitive pay, and a supportive work environment.

You will provide nursing care, administer meds, assist physicians, and collaborate with the RN and other staff to ensure high-quality resident care. Prior SNF experience and SigmaCare familiarity are preferred but not required.

Qualifications

• Active LPN license in New York state.
• CPR certification.
• Ability to observe and identify residents' needs and report to the RN/Supervisor.
• Prioritize and coordinate activities of the nursing unit.
• Demonstrated leadership abilities.
• Experience with SigmaCare is preferred.

Responsibilities

• Provides nursing care to patients as assigned.

• Assists Physicians and others with diagnostic procedures and treatments.
• Reinforces health care instructions to patients and families.

Duties & Responsibilities:

• Provides nursing care to patients as assigned.
• Administers and records medications and treatments as assigned.
• Prepares for and assists Physicians and others with diagnostic procedures and treatments.
• Reinforce supports and provides health care instructions to patients and their families.
• Participates with other team members in planning, implementing and evaluating nursing care and other patient activities.
• Assists in the admission, transfer, and discharge of patients.
• Observes and reports patients' vital signs, symptoms and general condition to the appropriate individual(s).
• Records on patient charts observation of patient's responses to nursing care rendered.
• Maintains environmental conditions conducive to the comfort and safety of patients.
• Assists with unit management and/or assume charge of unit as assigned.

Cobble Hill Health Center is a not-for-profit long-and short-term care facility providing services to the disabled and elderly for over 40 years.

Requirements:

• Active LPN license in the State of New York.
• CPR certification
• Have the ability to observe and identify the resident's needs, report to the RN/Supervisor and collaborate with the RN in making clinical decisions
• Prioritize and coordinate activities of the nursing unit
• Be an excellent problem solver and have strong decision-making skills
• Have demonstrated leadership abilities
• Have prior experience working in a skilled nursing facility (preferred, but not required)
• Experience with SigmaCare (preferred, but not required)
