Job summary

Trilogy Health Services, LLC seeks a dedicated Registered Nurse (RN) to lead a patient care team and deliver high-quality clinical care. The RN acts as a Team Leader, guiding staff through medications, treatments, and care planning in alignment with facility policies.

You will monitor residents’ status daily and coordinate with charge nurses and physicians as needed. The role emphasizes continuous assessment, documentation, and patient care across admission to discharge, within a long-term care

Qualifications

• Must have and maintain a current, valid state RN license.
• Current, valid CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure medications, treatments, documentation, family teaching, care planning and patient care conferences are compliant with policies and procedures.
• Use the nursing process to deliver patient care from admission through discharge and return to family/community.
