Job summary

Trilogy Health Services, serving Vincennes, IN, is seeking a Registered Nurse to provide direct, quality clinical care and to serve as a Team Leader for patient care staff. You will lead a team, manage medications and treatments, document care, and coordinate with families to support each resident’s well‑being.

You will apply the nursing process from admission through discharge, observe residents daily, and respond to changes in condition.

Qualifications

• Must have and maintain a current, valid state RN license.
• Current, valid CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure medicines, treatments, documentation, family teaching, care planning and patient care conferences.
• Utilize the nursing process in delivering patient care and ensure continuity of care from admission through discharge and return to family and community.
• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary.
