Job summary

Montefiore Hudson Valley Collaborative in New York is seeking an experienced Registered Nurse to join our acute care team. The RN will participate in performance improvement, maintain professional practice standards, and demonstrate leadership in patient care delivery.

The role requires coordinating interdisciplinary plans of care, documenting care plans, ensuring timely assessments, and educating patients and families. Strong critical thinking, communication and teamwork are essential.

Qualifications

• BS/BSN in Nursing required.
• NYS RN license with current registration in NYS required.
• Basic Life Support (BLS) training required.
• Certification in specialty required upon hire.
• PALS training upon hire.
• 1 year hospital or clinic experience preferred.
• Effective English communication skills.

Responsibilities

• Demonstrates leadership in coordination and delivery of patient care.
• Develops interdisciplinary plan of care and ensures 100% patients have plan of care.
• Ensures completion of patient care assignments 100% of the time.
• Documents teaching/learning needs of patients and families prior to discharge.
• Assesses patient progress and updates the plan of care as needed.
• Provides safe environment and administers medications per policy.
• Communicates with the care team and supports patient/family education.
