Job summary

Montefiore Hudson Valley Collaborative is seeking a Registered Nurse to participate in performance improvement, uphold professional practice standards, and demonstrate leadership in patient care. This role emphasizes coordinating interdisciplinary plans of care, ensuring documentation, and conducting timely admission assessments.

The position requires a BSN, NYS RN license, BLS, and preferred hospital/clinic experience.

Qualifications

• BS/BSN in Nursing required.
• NYS License for Registered Professional Nurse and current registration or valid permit to practice in NYS as an RN required.
• Basic Life Support (BLS) Training required.
• Certification in specialty Upon Hire required.
• NYS Mandated Infection Control Training and Child Abuse Reporter Training may be required.
• At least 1 year of hospital or clinic setting experience preferred.
• Effective English communication skills, both written and oral.

Responsibilities

• Demonstrates leadership in the coordination and delivery of patient care.
• Coordinates interdisciplinary plan of care and ensures 100% of patients have a documented plan of care.
• Completes admission assessments within 8 hours and documents with the interdisciplinary team.
• Participates in unit-based performance improvement and mentorship of associates.
• Engages in discharge planning and daily interdisciplinary rounds to guide care.
