Job summary

Montefiore Hudson Valley Collaborative seeks a Registered Nurse to lead and coordinate patient care within a multidisciplinary team at Montefiore Medical Center affiliated facilities. Qualifications

• RN requires Associate Degree in Nursing.
• BS in Nursing is preferred.
• Current NYS RN license and registration.
• 2+ years RN experience, Emergency Department preferred.

Responsibilities

• Demonstrates leadership in coordinating and delivering patient care.
• Coordinates an interdisciplinary plan of care.
• Ensures 100% of assigned patients have documented plans of care.
• Assesses and documents patient response to interventions and teaches plan of care.
• Communicates effectively with patients, families, and health care team.
• Maintains professional practice standards and ongoing competencies.
