Job summary

Montefiore Hudson Valley Collaborative is seeking a Registered Nurse to participate in coordination and delivery of patient care, lead interdisciplinary plans of care, and ensure documentation and discharge planning are completed within standard timelines.

The role emphasizes clinical leadership, adherence to standards of practice, patient safety, and effective communication with patients, families, and health care teams in a fast-paced environment.

Qualifications

• BS/BSN in Nursing required.
• NYS license as Registered Professional Nurse and current registration in NYS required.
• NYS Mandated Infection Control and Child Abuse Reporter Training required.
• Basic Life Support (BLS) Training required.
• Certification in specialty Upon Hire required.
• NYS Infection Control Certification.
• PALS Training Upon Hire.
• At least 1 year of Hospital or Clinic setting experience Preferred.
• Effective English communication skills, both written and oral.

Responsibilities

• Demonstrates leadership in the coordination and delivery of patient care.
• Coordinates an interdisciplinary plan of care and ensures 100% of assigned patients have a documented plan of care.
• Assesses health care of patients and documents teaching/learning needs prior to discharge and follows up.
• Demonstrates critical thinking to problem solve and initiate clinical decision making.
• Evaluates patient progress toward health goals and updates the plan of care accordingly while communicating in IDT rounds.
• Develops and provides direct patient care, incorporating clinical guidelines and patient-focused planning.
• Maintains a safe environment and administers medications according to policy.
• Communicates effectively with patients and health care team, upholding patient rights and privacy.
