Job summary

Home In The Heart Healthcare Agency is seeking a compassionate RN to conduct supervisory assessments and oversight visits for veterans receiving Homemaker/Home Health Aide services. The role covers the MORRIS CHAPEL, TN service area and requires strong clinical skills to guide caregivers and ensure quality care.

You will perform initial assessments, develop care plans, and ensure compliance with agency and VA regulations while communicating with families, physicians, and case managers.

Qualifications

• Current, unrestricted Tennessee RN License.
• Valid driver’s license and reliable transportation.
• Strong clinical assessment and documentation skills.
• Excellent communication and organizational skills.
• Ability to work independently and travel within the service area.

Responsibilities

• Evaluate client health, functional needs, safety, and home environment.
• Develop and update individualized care plans.

Service Area: MORRIS CHAPEL, TN

Key Responsibilities

• Conduct initial assessments, supervisory visits, and reassessments.
• Evaluate client health, functional needs, safety, and home environment.
• Develop and update individualized care plans.
• Provide clinical guidance and oversight to caregivers.
• Communicate with clients, families, physicians, case managers, and VA representatives.
