Job summary

Home In The Heart Healthcare Agency is seeking a compassionate RN to conduct supervisory assessments and oversight visits for veterans receiving Homemaker/Home Health Aide services. This RN visit occurs monthly and requires robust clinical judgment and coordination with care teams.

The role will involve travel within the service area, adherence to regulatory requirements, and providing guidance to caregivers. Competitive per-visit compensation and mileage reimbursement are offered.

Qualifications

• Current, unrestricted Tennessee RN License
• Valid driver's license and reliable transportation
• Strong clinical assessment and documentation skills
• Excellent communication and organizational skills
• Ability to work independently and travel within the service area

Responsibilities

• Conduct initial assessments, supervisory visits, and reassessments.
• Evaluate client health, functional needs, safety, and home environment.
• Develop and update individualized care plans.
• Provide clinical guidance and oversight to caregivers.
• Communicate with clients, families, physicians, case managers, and VA representatives.
• Complete accurate and timely documentation.
• Ensure compliance with agency, VA, and Tennessee regulations.
