Job summary

Home In The Heart Healthcare Agency seeks a compassionate Registered Nurse (RN) to conduct supervisory assessments and oversight visits for veterans receiving Homemaker/Home Health Aide services in Hardin County and surrounding areas.

This PRN role requires strong clinical assessment, documentation, and the ability to travel within the service area, providing guidance to caregivers and collaborating with families, physicians, case managers, and VA representatives.

Qualifications

• Current, unrestricted Tennessee RN License.
• Valid driver’s license and reliable transportation.
• Strong clinical assessment and documentation skills.
• Excellent communication and organizational skills.
• Ability to work independently and travel within the service area.

Responsibilities

• Conduct initial assessments, supervisory visits, and reassessments.
• Evaluate client health, functional needs, safety, and home environment.
• Develop and update individualized care plans.
• Provide clinical guidance and oversight to caregivers.
• Communicate with clients, families, physicians, case managers, and VA representatives.
• Complete accurate and timely documentation.
• Ensure compliance with agency, VA, and Tennessee regulations.

Home In The Heart Healthcare Agency is seeking a compassionate and experienced Registered Nurse (RN) to conduct supervisory assessments and oversight visits for veterans receiving Homemaker/Home Health Aide services.

Service Area: Hardin County and surrounding areas

Key Responsibilities

• Conduct initial assessments, supervisory visits, and reassessments.
