Job summary

Hand to Hand Homecare & Transportation in Virginia is seeking an experienced Registered Nurse (RN) Clinical Supervisor to provide clinical oversight for personal and respite care. This hybrid, part-time role involves client assessments, care plan development, and supervising direct care staff.

You will ensure compliance with state and Medicaid requirements, complete documentation, and support quality care delivery. Travel to client locations within the service area is required.

Qualifications

• Virginia RN license or multistate privilege required.
• Minimum one year of clinical nursing experience in a qualifying healthcare setting.
• Experience with personal care assessments, care planning, documentation and staff supervision.
• Strong clinical judgment, organizational and communication skills.

Responsibilities

• Conduct initial and ongoing client assessments.
• Develop and update individualized care plans.
• Supervise aides and LPNs to ensure quality care.
• Complete clinical documentation and maintain accurate records.
• Perform supervisory visits and address client care concerns.
• Participate in quality improvement activities and compliance with regulations.
• Collaborate with clients, families, caregivers and agency leadership to promote positive outcomes.
