Job summary

The Southern Touch Home Care LLC in Georgia is seeking a compassionate PRN Registered Nurse (RN) to support our growing home care team. This field-based role serves adults in clients' homes, assessing needs and delivering skilled nursing services with independence.

You will conduct admissions, develop care plans, perform supervisory visits, and educate clients and caregivers while maintaining clinical documentation and communicating changes to the care team.

Qualifications

• Active, unrestricted Georgia RN license.
• Experience caring for adults with various diagnoses.
• Home health/home care nursing experience strongly preferred.
• Experience with veterans, dementia, cognitive impairment, chronic conditions, post-hospitalization care, or mobility limitations is a plus.
• Strong assessment, documentation, and communication skills.
• Able to work independently in home environments.

Responsibilities

• Complete admission and nursing assessments.
• Assess medical, functional, personal care, and safety needs at home.
• Develop and update plans of care.
• Perform RN supervisory visits.
• Provide intermittent skilled nursing services as ordered.
• Document accurately and timely clinical information.
• Communicate concerns to the care team.
• Educate clients and caregivers as appropriate.

Job description

The Southern Touch Home Care is seeking a compassionate and experienced PRN Registered Nurse (RN) to support our growing home care agency.

This field-based position is ideal for an RN with experience caring for adults with a wide range of medical conditions and care needs, including Veterans, older adults, individuals with dementia or cognitive impairment, and clients recovering from hospitalization, illness, or injury.

Home health or home care experience is strongly preferred, as this role requires an RN who is comfortable working independently in clients’ homes, assessing the home environment, identifying safety and care needs, and making sound clinical recommendations.

Key Responsibilities

• Complete comprehensive admission and nursing assessments
• Assess medical, functional, personal care, and safety needs within the home
• Develop and update plans of care
• Complete required RN supervisory visits
• Identify changes in condition and determine when additional intervention or follow-up may be needed
• Provide intermittent skilled nursing services as ordered
• Complete accurate and timely clinical documentation
• Communicate concerns and changes in condition to the appropriate care team
• Provide client and caregiver education as appropriate

Qualifications & Preferred Experience

• Active, unrestricted Georgia Registered Nurse (RN) license
• Experience caring for adults with a variety of diagnoses and care needs
• Home health or home care nursing experience strongly preferred
• Experience with Veterans, dementia, cognitive impairment, chronic conditions, post-hospitalization care, or mobility limitations is a plus
• Strong assessment, documentation, and communication skills
• Comfortable working independently in a variety of home environments
• Reliable transportation
• Ability to meet all agency background, health screening, and compliance requirements
