Job summary

Constructive Partnerships Unlimited seeks a Registered Nurse Supervisor to lead nursing assessments, develop care plans, and oversee LPNs ensuring quality care for individuals with disabilities within our supported living programs.

The role emphasizes collaboration with physicians, training staff, documenting care, and coordinating follow‑ups to prevent hospitalizations while maintaining compliance with applicable laws and agency policies.

Qualifications

• NYS RN with BSN minimum license.
• Minimum 2 years experience working with persons with disabilities.
• Extensive knowledge of pertinent health resources.
• CPR certification required.

Responsibilities

• Assess individuals to determine needs, resources, and health outcomes.
• Develop and update health care plans; provide direct nursing services.
• Oversee plan implementation; train staff in care plans.
• Evaluate plan effectiveness and communicate outcomes.
• Refer to other health providers and schedule follow-ups.
• Ensure documentation of care, medications, and visits.
• Facilitate conferences to resolve complex issues with team.
• Maintain communication with physicians, supervisors, and clinicians.
• Collaborate across teams from admission to discharge.
• May accompany individuals to clinics or outings.
• Develop strategies to prevent hospital admissions and deteriorations.
• Review clinic notes and implement orders timely.
• Update patient profiles with Residential Coordinator and providers.
• Review medication administration procedures and ensure compliance.
• Provide direct oversight to LPNs on shift; report issues to Charge Nurse.
• Supervise Direct Care staff in absence of coordinators.
• Coordinate with Day Program Nurse to meet medical needs.
