Job summary

Sentara Norfolk General Hospital in Norfolk, VA is seeking a full-time, temporary Registered Nurse Stroke Program Coordinator/Navigator to drive patient progression and improve outcomes for stroke patients.

This 8-month assignment, with potential to become permanent, requires BSN, RN license, BLS within 90 days, and 3 years RN experience. You will lead cross-disciplinary care and support timely discharge planning and transitions.

Qualifications

• BSN required and RN license.
• BLS certification within 90 days of hire.
• Minimum 3 years RN experience.

Responsibilities

• Daily patient surveillance to support timely progression of care across units.
• Identify and escalate delays related to diagnostics, consults, procedures, discharge planning, or clinical changes.
• Recognize high-risk patients early and elevate concerns related to decline, complications, or delays.
• Coordinate care across neurology, hospitalists, rehab, case management, nursing and other partners.
• Support early discharge planning for transitions to rehab, SNF, home health, hospice or other settings.
• Facilitate early goals-of-care and palliative integration for high-risk populations.
