Qualifications

• Bachelor's degree in nursing required.
• RN licensed in Virginia or eligible.
• CPR/BLS certification required.

Responsibilities

• Assess patients and families to determine needs and resources.
• Collaborate with case management for discharge planning.
• Perform utilization review of stroke patient records.
• Develop and improve the stroke program.
• Build relationships with stroke patients, team, and physicians.
• Coordinate referrals and education for patients and caregivers.
• Maintain dashboards reflecting stroke service indicators.
• Schedule appointments and ensure quality patient care.
