• Provides supportive resources and education to patients and families on treatment and side effects.
• Teaches patients and families about treatment expectations and procedures.
• Completes patient forms such as FMLA, disability, and letters of medical necessity.
• Assists benefits team in obtaining authorizations for chemotherapy and infusion-related treatments.
• Facilitates communication via voicemails, emails, and Teams messages.
• Develops and implements care plans for oncology patients and coordinates care.
