Heart to Heart Hospice in the Houston area is seeking a dedicated Night Shift RN to observe and assess residents, document changes, and report findings. You will implement the Plan of Care and respond to evolving patient needs according to policy.

This role emphasizes compassionate care during night rounds with a focus on quality outcomes. Requirements include an active RN license, a valid driver's license with reliable transportation, and 2 years of RN experience in healthcare settings.

Qualifications

• Current State Registered Nurse License (RN).
• Driver's license and reliable transportation required.
• Two years of RN experience preferred in a healthcare/clinical setting.

Responsabilités

• During rounds observes, assesses, documents and reports resident conditions and changes.
• Implement the individualized Plan Of Care as directed.
• Identify significant changes in residents' condition and take action per facility policies and procedures.

What You Must Have:

• Current State Registered Nurse License
