Heart to Heart Hospice in Texas is seeking a Registered Nurse On Call to deliver compassionate in-home care, perform assessments, and provide support during patient crises.

You will develop and revise nursing care plans with the physician and IDG, document progress, and ensure compliance with Medicare and internal policies while maintaining excellent communication with patients and families.

Qualifications

• RN license in good standing
• Two years RN experience preferred in healthcare/clinical setting
• Current driver's license and reliable transport with ongoing auto liability insurance

Responsabilités

• Provide home visits for patient assessment, treatment and support, including pronouncement of death when needed
• Develop and revise comprehensive nursing care plans with patient, physician and IDG
• Maintain excellent customer care and phone etiquette with internal and external customers
• Prepare progress notes showing goals progress and outcomes
• Inform physician of changes in patient needs and outcomes of interventions
• Ensure timely submission of all documentation and compliance with laws and policies
• Perform other duties as required and maintain Medicare compliance

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:

• Provide home visits as needed for patient assessment, treatment, and support. Visits may include pronouncement of patient death, crisis management and admission
• Plans for comprehensive service to patients and their families; develops and/or revise nursing care plans, including goals in conjunction with the patient’s physician and IDG
• Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette
• Prepare clinical and progress notes that demonstrate progress toward established goals
• Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
