Job summary

AdvantageCare Physicians is seeking a Registered Nurse (Care Team Supervisor, Specialty) at Clove Road to oversee the specialty care team, supervise clinical staff, and coordinate patient care within our New York-based offices. You will ensure adherence to patient safety standards, educate patients, and act as a liaison among patients, staff and physicians.

The role requires nursing licensure, BLS, and ACLS for GI and Cardiology with prior hospital or ambulatory care experience.

Qualifications

• Graduate of an accredited school of nursing.
• Bachelor’s degree in nursing preferred.
• BLS certification required.
• ACLS required for GI and Cardiology.
• Experience in hospital or ambulatory settings preferred.

Responsibilities

• Provide professional nursing care to patients and administer medications per orders.
• Serve as liaison between patients, staff and physicians and support the Advanced Care Model.
• Organize and provide clinical supervision to the specialty patient care team.
• Coordinate and manage daily huddles and prepare materials and deliverables.
• Review patient charts for missing labs, reports, and open orders and review as part of care team huddles.
