• Collaborate with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive care to patients in their homes and conduct patient home visits.
• Assess, plan, direct, and evaluate each patient’s care plan to meet their needs.
• Manage emergency situations promptly and professionally.
• Complete medication administration and treatment needs for all assigned patients.

Responsibilities

What You Will Do:

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
