Job summary

Immediate Home Health and Hospice in Philadelphia is seeking caring professionals to join our team to deliver comprehensive home-based care. You will work closely with physicians, nurses, and therapists to assess, plan, and execute patient care plans in patients' homes.

You will respond to emergencies promptly and administer medications as needed, ensuring safe and effective treatment for our patients. We offer a sign-on bonus and a supportive environment focused on compassionate care.

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
