Job summary

CareRing Health is actively seeking a dedicated Registered Nurse for home health and hospice services in the Philadelphia region. You will conduct patient home visits, assess and direct care plans, and respond to emergencies with professionalism and precision.

Ideal candidates bring an active RN license, reliable transportation, and at least one year of nursing experience, preferably in home health with OASIS exposure. EMR proficiency and knowledge of regulations strengthen your application.

Qualifications

• Current, active Registered Nurse (RN) license.
• Valid driver's license and reliable transportation.
• At least one year of nursing experience; home health and OASIS experience preferred.
• Knowledge of OASIS, Medicare Conditions of Participation, and home health regulatory requirements.
• Strong assessment, critical thinking, and time management skills.
• Proficient with electronic medical records (EMR); WellSky Kinnser experience is a plus.
• Eligibility for certain benefits may depend on employment status.

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis
