Qualifications

• Active NY RN license.
• Minimum 2 years SNF experience preferred.
• Strong clinical assessment and critical thinking.

Responsibilities

• Provide exceptional nursing care while ensuring the health, safety, and well-being of residents.
• Collaborate with DON, ADON, physicians, and interdisciplinary team to deliver high-quality care.
• Supervise, mentor, and support nursing staff to maintain excellence in resident care.
• Administer medications and treatments accurately per physician orders and policies.
• Assess resident conditions, monitor changes, and respond to medical needs.
• Maintain accurate documentation in compliance with regulations.
• Communicate effectively with residents, families, and team members.
• Assist with admissions, transfers, and discharges while ensuring continuity of care.
• Promote a positive, resident-centered environment.
• Ensure compliance with infection control, safety, and regulatory standards.
