Thompson Health in Canandaigua, NY is seeking a part-time RN for the Skilled Transition Unit in Continuing Care Center. You will work evenings with weekend/holiday requirements, providing skilled nursing care and leading unit operations with a focus on resident-centric care.

Qualifications

• Graduate of approved RN nursing program with AAS or BSN.
• Valid NYS license as a RN.

• 1 year or more in Long Term Care or acute care preferred.
• Experience guiding nursing through regulatory surveys.
• Excellent organizational skills.

Responsibilities

• Provide skilled nursing care for residents and oversee daily unit operations.
• Work with the interdisciplinary team to develop a comprehensive resident care plan.
• Supervise and assist with daily tasks and care for residents.
• Document changes and factors related to resident condition.

Main Job Function:

Provide skilled nursing care for residents and oversee daily operation of Unit (Avenue)

Key Responsibilities:

• Works with the interdisciplinary team to develop a comprehensive approach to resident care
• Supervise and assist with daily tasks and care for residents
• Develop care plans for residents in a timely fashion in conjunction with associates and Nurse Manager
• Document all changes and factors as related to resident condition
• Identify and report to Nurse Manager any concerns with residents or with Policy and Procedure

Requirements:

• Graduate of approved RN nursing program with AAS or BSN
• Valid NYS license as a RN
• BLS
• 1 yr. experience in Long Term Care or acute care desired but not required
• Experience with guiding nursing through the regulatory survey process
• Excellent organizational skills

Position Pay Range: $38.00-50.00/hour

Starting Pay: Based on experience
