Job summary

Polaris Rehabilitation and Care Center in Cheyenne, WY is seeking a compassionate Registered Nurse to provide skilled nursing and rehabilitation care. Relocation bonus is available for eligible nurses ready to make Cheyenne their home.

The RN will assess residents, administer medications, and develop care plans while coordinating with a multi-disciplinary team—building relationships with residents and families.

Qualifications

• Active RN license required, Wyoming.
• Current CPR/BLS certification.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.

Responsibilities

• Assess residents, recognize changes in condition, and coordinate timely follow-up with providers.
• Administer medications and treatments according to orders and facility procedures.
• Develop, implement, and evaluate individualized care plans.
• Provide direction and support to nursing assistants and other care team members.
• Maintain accurate, timely clinical documentation.
• Communicate with residents and families about care, progress, and concerns.
• Promote resident dignity, safety, infection prevention, and quality care.
