Job summary

CareAparent is seeking a Registered Nurse Case Manager (RNCM) to conduct in-home assessments, develop and monitor care plans, and supervise home health aides. The role supports seniors to stay in their homes with safe, compassionate care.

You will educate patients and families, collaborate with physicians and therapists, and oversee skilled nursing tasks and visits while coordinating with the Clinical Manager. Travel is required across local zones.

Qualifications

• Excellent customer relations and interpersonal skills.
• Excellent verbal/written communication skills.
• Ok with travel to and from client homes.
• Ability to work on call for rotating weekends and holidays.
• Possess a valid driver's license and insured personal vehicle.

Responsibilities

• Conduct initial and ongoing in-home nursing assessments and develop care plans.
• Perform medication set-ups and supervise home health aides and CNAs.
• Coordinate care with interdisciplinary teams and physicians.
• Oversee and supervise LPN visits and ensure plan of care is followed.
• Prepare visit reports and patient records using EMR systems.
