Position: RN – Resource Pool – Transitional Care Unit Support (Open).
As a registered nurse, you are expected to deliver the best in care and customer service, demonstrating respect, dignity, kindness, and empathy in each encounter with patients, families, visitors, and colleagues. You will provide nursing coordination and individualized care for patients of all ages and collaborate with the health care team to deliver high‑quality, patient‑centered services.
Assess, plan, implement, and evaluate patient care to achieve optimum patient outcomes for patients of all ages as appropriate to the service area.
Complete age‑specific admission and ongoing assessments, document findings, and use the information to develop, implement, and evaluate an interdisciplinary plan of care. Revise the plan according to changes in patient status.
Accurately administer and document treatments and medications via automated computer systems, including electronic medical records.
Identify patient/family educational needs in a timely manner, incorporate them into the plan of care, and document appropriately.
Contribute to Carilion Clinic’s performance improvement activities.
Act as a patient advocate, protector of patient rights, and provider of psychological/social/cultural support.
Apply knowledge of growth and development to appropriately care for patients served.
Plan strategies to reduce length of stay, turnaround time, and resource consumption while improving patient outcomes.
Serve as a role model for clinical practice to coworkers, students, and physicians.
Manage resources, establish priorities, and modify the environment to meet patient care needs.
Complete manager/director predetermined projects within the specified time frame and according to pre‑established guidelines.
Use problem‑solving methods to resolve patient and personnel situations within the work area.
Take an active role in the orientation of new employees and provide feedback to staff using appropriate communication and documentation.
Provide input into practice, environment, and work issues by participating on unit, hospital, and/or system activities, facilitating staff participation and input in decision making.
Assist with educational activities for the unit/department.
