Job summary

Cascade-Manor in Eugene, OR is seeking a Resident Care Manager - RN to lead a skilled nursing team and deliver person-centered care that enhances health, independence, and quality of life. You will coordinate across physicians, nurses, therapists, and other care professionals, oversee admissions, care planning, and regulatory readiness, and mentor staff to ensure compassionate, compliant care.

Requirements include an active RN license, 1 year of nursing experience in a skilled nursing setting,

Qualifications

RN license required.
1 year of professional nursing experience in a skilled nursing setting.
Prior supervisory experience in a similar setting.

Responsibilities

Lead and inspire a high-performing nursing team.
Advocate for resident rights and deliver resident-centered care.
Coordinate care across physicians, nurses, therapists, and the interdisciplinary team.
Oversee admissions, assessments, care planning, conferences, and discharge planning.
Monitor resident conditions and clinical documentation for high standards of care.
Drive quality improvement initiatives and regulatory readiness.
Partner with families and healthcare representatives for collaborative care planning.
Support MDS coordination, PDPM and OBRA compliance, and reimbursement processes.
Mentor and develop nursing staff; provide on-call leadership as needed.
