Qualifications

• Graduate of an accredited nursing school.
• Current Connecticut RN license.
• BLS certification by the American Heart Association.

Responsibilities

• Implements and monitors patient care plans and communicates patient condition.
• Acts as primary coordinator of all disciplines for well-coordinated patient care.
• Notes and carries out physician and nursing orders.
• Coordinates patient discharge planning with the healthcare team.
• Assesss patient needs including psychosocial, cultural, spiritual and age-related aspects to provide individualized care.
• Delegates care to nursing staff to maximize performance.
• Participates in daily interdisciplinary patient rounds to inform care decisions.
• Adheres to ANA standards and ethics in nursing practice.
• Develops, implements and revises individualized plans of care.
• Engages in professional growth activities.
