Job summary

Gundersen Health System in La Crosse, WI is seeking a Pulmonary Nodule Navigator RN to coordinate care for patients with pulmonary nodules across the care continuum. You will educate patients and families, provide procedural education, and manage communications as part of a collaborative multidisciplinary team.

The role requires an Associate Degree in Nursing and current WI RN licensure; licensure in MN and IA within 3 months is expected. BLS certification is also required.

Qualifications

• Associate degree in nursing required.
• Current Wisconsin RN licensure upon hire.
• RN licensure in Minnesota and Iowa within 3 months of hire.
• BLS for Healthcare Providers required.

Responsibilities

• Coordinate care for patients with pulmonary nodules across the continuum of care.
• Educate patients and families regarding diagnoses and follow-up
• Provide procedural education and instructions.
• Perform telephone triage and clinical assessment.
• Manage patient communications through phone calls and inbox messaging.
• Collaborate with physicians and care teams to facilitate timely care.
