Job summary

Spring Valley Hospice in Tupelo, MS is looking for a Hospice Admissions RN to provide expert, patient-centered care during critical times. You will visit patients in various settings, explain services, and conduct clinical assessments to determine hospice eligibility.

Qualifications

• Graduate of an accredited nursing school with current licensure.
• One year of hospice nursing experience required; 3+ years preferred.
• Licensed driver with an insured automobile in good working order.

Responsibilities

• Visit patients referred for hospice care.
• Explain hospice services and obtain necessary consents.
• Perform comprehensive clinical assessments for eligibility.
• Coordinate the start of nursing and aide services.
• Collaborate with interdisciplinary team and maintain documentation.

As a Hospice Admissions RN You will

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required; 3+ years preferred, or similar setting such as Long-Term Care, Intensive Care, or Emergency Room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and is in good working order
