Job summary

BrightSpring Health Services, through its Adoration Health affiliate, is seeking a Hospice Admissions RN to join our Chattanooga, TN team. The role involves visiting patients in homes and facilities, coordinating hospice services, and working with an interdisciplinary team to ensure compassionate, patient-centered care.

The position requires a nursing degree and state licensure, with preferred hospice experience.

Qualifications

• Graduate of an accredited nursing program with valid state licensure.
• One year hospice nursing experience preferred; other acute care settings considered.
• Must have reliable transportation and valid driver license.

Responsibilities

• Visit patients referred for hospice care in homes, hospitals, or facilities.
• Explain services, discuss goals, and obtain consents and required documents.
• Collect medical history, medications, and symptoms; assess eligibility per Medicare/insurance guidelines.
• Coordinate start of services and collaborate with the interdisciplinary team.
• Maintain accurate, timely documentation of care and visits.

As a Hospice Admissions RN You will:

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications.)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
