Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Hospice RN Case Manager in Cleveland, MS. You will assess patient needs, develop and implement individualized hospice care plans, administer care, and support families in collaboration with the interdisciplinary team.

We offer flexible scheduling, strong job stability, and opportunities for advancement. Ideal candidates have hospice experience, licensure, and a valid driver’s license for community-based care.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Must be a licensed driver with a vehicle that is insured and in good working order

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch
