Responsibilities

What You’ll Do:

• Complete a two-week clinical orientation covering procedures, EMR system, safety protocols, and compassionate care.
• Assess patients with complex health needs and develop personalized care plans.
• Educate and guide patients and families through their healthcare journey.
• Collaborate with VNA clinicians and external providers to ensure seamless care.
• Document patient conditions and update care plans accordingly.

Qualifications

What We’re Looking for in You:

• Registered Nurse (RN) with a Bachelor’s Degree in Nursing (preferred).
• At least 1 year of clinical experience as a registered nurse; homecare experience is a plus.
• Strong communication skills for guiding and educating patients and families.
• Proficiency with EMR systems and digital tools.
• Reliable transportation, a car, and a valid driver’s license for home visits.
