The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in compliance with laws, regulations, and policies.

Responsibilities include:

1. Coordinating care plans with healthcare professionals and ensuring continuity of care through effective communication.
2. Conducting initial and comprehensive nursing evaluations, developing care plans within state guidelines, and documenting accurately and timely.
3. Performing patient assessments and developing appropriate care plans.
4. Ensuring patients meet home health eligibility and medical necessity guidelines.

License Requirements

• Current RN license in the practicing state
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to reliable transportation
