RN Registered Nurse Float Pool FT Nights

Shift: Night

Manages individualized, outcome-oriented nursing care through the use of the nursing process. Provides care, treatment, and services through the successful coordination and completion of a series of processes that include assessment, planning care, providing care, in accordance with departmental and hospital policies and procedures.

1 - ASSESSMENT/REASSESSMENT
Assesses and diagnoses patient and family needs in order to provide quality care to assigned patients by:

  1. Initiating admission assessment within 8 hours of admission, identifying and documenting patient/family/significant other teaching needs upon admission and throughout hospitalization.

2 - PLAN OF CARE
Develops, discusses, and communicates a prioritized problem list/plan of care for each patient, in collaboration with designated support persons in order to address all identified needs.

  1. Develop, evaluate, and update individualized plan of care for patients and document outcomes.
  2. Actively communicate plan of care with the primary nurse assigned to care for the patient.
  3. Facilitate discharges/transfers by goal of 12:00 pm, communicating barriers utilizing the Chain of Command.
  4. Document all patient care following the department of nursing policies, unit-based standards, and nurse-sensitive indicators (falls, skin pain).
  5. Assess/reassess and document patient's level of comfort utilizing the appropriate pain scale and the patient's response to each intervention both pharmacological and non-pharmacological per South Shore Hospital Policy.
  6. Work on behalf of patient/family. Seek help to represent patient/family when they are unable to represent themselves.
  7. Raise ethical questions and concerns with clinical team. Seek available resources to help formulate and understand ethical decisions.

3 - MEDICATION ADMINISTRATION

  1. Demonstrate knowledge of and follow SSH policies and procedures for administering, transcribing, and recording medications.
  2. Demonstrate proper procedure for the documentation of narcotic withdrawal, administration, verifies count, wastes per policy and resolves narcotic discrepancy.
  3. Initiate and maintain IV access according to SSH policy and procedure.

6 - SAFETY
Fosters a "Culture of Safety" through personal ownership and commitment to a safe environment.

  1. Verify patient identification with two identifiers prior to the start of any invasive procedure, including "time out", administration of care, medications, labeled specimens and document confirming the correct patient, procedure, site, equipment, and consent.
  2. Comply with the current CDC hand hygiene guidelines through proper handwashing. Adhere to universal precautions, make appropriate use of personal protective equipment at all times, and appropriately dispose of hazardous materials. Maintain awareness of MSDS sheets and how to access them.
  3. Assess each patient's risk for falling throughout hospitalization using appropriate interventions to prevent falls.
  4. Ensure environment meets regulatory requirements at all times (e.g., no cardboard boxes on floor, equipment cords are secured and plugged into foot of bed, etc.).
  5. Understand and be able to demonstrate individual roles and responsibilities in the event of hospital codes/emergency preparedness.
  6. Practice patient handoff utilizing the principles of SBAR - including walking rounds.

7 - PROFESSIONAL DEVELOPMENT
Assumes overall responsibility for own professional development by incorporating evidence-based practice, research, and performance improvement initiatives as a part of ongoing nursing practice.

  1. Obtain at least 5 contact hours per year in the area of practice.
  2. Practice within the legal boundaries of MA Nurse Practice Act. Direct other licensed and non-licensed personnel as assigned.
  3. Active participant in unit-based initiatives and/or hospital committees.
  4. Understand and demonstrate job responsibilities for charge nurse, preceptor, and mentor role.
  5. Analyze cause of problems, identify and evaluate alternative solutions, and select appropriate solutions and communicate appropriately and effectively utilizing the proper Chain of Command.
  6. Assume an active role in keeping informed about changes in hospital policy, procedure, and equipment.
  7. Interact with patients and families in a compassionate and humanistic manner - demonstrating empathy, sensitivity, and patience.

8 - TECHNOLOGY
Embrace technological solutions to work processes and practices.

  1. Access Lotus Notes to review e-mail, learning management, and other resources as applicable to RN role.
  2. Utilize all Meditech applications required by department and unit standards (PCS, EMR, Order Entry, etc.).

JOB REQUIREMENTS

Minimum Education: Graduate of an accredited School of Nursing

Minimum Work Experience: Previous Acute Care RN experience preferred

Required Licenses / Registrations: RN - Registered Nurse

Required Classes/Skills: ACLS - Advanced Cardiac Life Support

CAP Education Verification Required

Required additional Knowledge, and Abilities: Demonstrated proficiency in acute care nursing, knowledge, and skills

N12x3, 7p-730am, every 3rd weekend and every other holiday. This is a great way to gain experience on a variety of Med/Surg units, and our Cardiac floor. Our Float Pool RNs make an additional $5/hour "float bonus" every shift!

License/Registration/Certification Requirements: ACLS (AHA) Advanced Cardiac Life Support Certification - American Heart Association (AHA) (Including courses offered through SSH), INSTRUCTOR- Advanced Cardiac Life Support (ACLS) - American Heart Association (AHA) (Including courses offered through SSH), RN-Registered Nurse - Board of Registration in Nursing (Massachusetts)
