Job summary

Generations Hospice in Wisconsin seeks a dedicated Hospice RN Case Manager to deliver compassionate, patient-centered care across Beaverdam, Mayville, and Fond du Lac. This full-time role includes a $7,500 sign-on bonus and a structured schedule from 8 AM to 5 PM, Monday through Friday.

You will work within a strong interdisciplinary team, ensure comprehensive care plans, and support families while maintaining thorough documentation. A valid RN license and hospice experience are preferred.

Qualifications

• Graduate of an accredited nursing school with current RN licensure.
• 1 year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Must be a licensed driver with an automobile that is insured.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
