Job summary

Adoration Health is seeking a compassionate RN Case Manager for Hospice in Parkersburg, WV. This role provides patient-centered care across the hospice spectrum, coordinating plan development and direct nursing tasks to ensure comfort and dignity for patients and families.

You will work with an interdisciplinary team, manage documentation and care plans, and participate in on-call rotations. A nursing license in the state and CPR certification are required, with priority given to hospice

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• Minimum 1 year nursing experience; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must have a driver’s license and CPR certification where required.
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients' families and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications

• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
