Job summary

Adoration Health, part of BrightSpring Health Services, is seeking a compassionate RN Case Manager for Hospice in Parkersburg, WV. This full-time role covers a regional hospice team with a schedule Monday–Friday, 8am–5pm, and opportunities to impact patients and families at a critical time.

You will assess needs, develop care plans, administer treatments, and collaborate with an interdisciplinary team to ensure clinical excellence and smooth documentation in a supportive work environment.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patients wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch
