Job summary

Adoration Hospice, part of Adoration Health, seeks a Registered Nurse Case Manager to deliver patient-centered hospice care in Parkersburg, WV. The role focuses on comprehensive assessments, care planning, and direct nursing care in a supportive team environment.

You will coordinate with families and interdisciplinary staff, ensure documentation accuracy, and participate in on-call rotations. A nursing degree and WV licensure are required, with preferred hospice experience.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must be a licensed driver with an automobile in good working order.
• Possess and maintain current CPR certification as required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patients' wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
