As a RN Case Manager You will:

• Assess and monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient’s wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on‑call rotation as required by local branch

Qualifications

Additional Job Information

Adoration Hospice / BrightSpring Health Services, an affiliated company who employs those individuals working for Adoration Hospice recently acquired certain LHC Group home health and hospice operations and is recruiting candidates for open positions within those operations. The successful candidate will initially be employed by LHC Group until January 1, 2026 at the latest. During this period, LHC Group will perform all administrative onboarding activities and offer benefits coverage under LHC Group employee benefit plans. On or before January 1, 2026, the successful candidate’s employment will be transferred to Adoration Hospice / BrightSpring Health Services, an affiliated company who employs those individuals working for Adoration Hospice.
