Adoration Health in Parkersburg, WV is seeking a compassionate RN Case Manager to join our hospice team. You will assess patients, develop and adjust care plans, administer treatments, manage pain and symptoms, and collaborate with an interdisciplinary team.

This full-time role offers 1:1 patient care, flexible scheduling, and opportunities for professional growth in a supportive environment. A valid RN license in WV, CPR certification, and hospice or similar experience are preferred.

Qualifications

• Graduate of an accredited nursing program with current licensure to practice in the state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must have a valid driver's license and insure vehicle as required.
• Maintain CPR certification where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patients' wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications
