Adoration Health in Parkersburg, WV is seeking a dedicated RN Case Manager for hospice care. This full-time role covers Wood, Pleasants, Ritchie, Wirt, Calhoun, and Roane Counties with a Monday–Friday, 8am–5pm schedule.

You will assess patients, create care plans, oversee medications and symptom management, educate families, collaborate with the inter-disciplinary team, and maintain timely documentation while participating in on-call rotation.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in state of operation.
• One year RN experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient families and caregivers.
• Collaborate with interdisciplinary team and maintain timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
