Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a dedicated RN Case Manager for Hospice in Parkersburg, WV. You will provide 1:1 patient-centered care, develop and coordinate hospice plans, administer medications, and support families, with on-call rotation as needed.

Requirements include WV nursing licensure, at least 1 year of nursing experience (3+ years preferred), hospice experience preferred, and a valid driver's license with a reliable vehicle.

Qualifications

• Graduate of an accredited nursing program with current licensure in WV.
• 1 year of nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care and documentation excellence.
• Must have a valid driver's license and insured vehicle; CPR certification where required.

Responsibilities

• Assess and monitor physical, emotional, and physiological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient families and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation

• One year nursing experience required , 3+ years preferred

• Experience in hospice or a similar setting preferred

• Knowledge of the hospice philosophy of care

• Commitment to clinical and documentation excellence

• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

• Possess and maintain current CPR certification in states where required.
