Job summary

Adoration Health is seeking a Hospice RN to join our Independence, OH team. You will provide expert, patient-centered care during a critical time in patients' lives and work in a supportive environment that values empathy and clinical excellence.

The role emphasizes independence with team collaboration, on-call support as needed, and a focus on comprehensive hospice care plans. Relevant nursing licensure in OH and hospice experience are preferred.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a vehicle insured and in good working order.

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
