Job summary

Adoration Health in Indianapolis, IN is seeking a compassionate Hospice RN Case Manager to assess patient needs, develop individualized care plans, and administer nursing care at patients’ homes or facilities. You will collaborate with an interdisciplinary team to ensure comfort and dignity at end of life.

Ideal candidates hold an active RN license in Indiana, have 1 year of nursing experience (3+ preferred), hospice exposure, and CPR certification where required; valid driver’s license and

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with patient wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required , 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
