Adoration Health in Ashtabula, OH is seeking a compassionate Hospice RN to join our team. You will provide patient-centered care at a critical time, ensuring comfort, dignity, and support for patients and families.

Qualifications

• Graduate of an accredited nursing school with current licensure.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must be a licensed driver with an automobile in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient’s wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on‑call rotation as required by local branch
