Job summary

Hospice of North Alabama seeks an experienced RN Case Manager to deliver patient-centered hospice care in Cullman, AL. You will assess needs, develop care plans, administer treatments, and collaborate with an interdisciplinary team.

The role offers independent work with team support, on-call rotation, and meaningful impact on patients and families; opportunities for growth within a growing hospice network.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patients wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Office Location: Cullman, AL

Coverage area: Morgan County and areas of Lawrence County ( Moulton, Hillsboro, Trinity )

Schedule: Monday thru Friday, 8am-5pm

As a RN Case Manager You will:
