Hospice of North Alabama is seeking a Registered Nurse Case Manager to join our Cullman, AL team. This role focuses on delivering compassionate, patient-centered hospice care, creating personalized care plans, and supporting families throughout the patient’s journey.

Ideal candidates will have an Accredited RN license, 1 year of nursing experience (3+ years preferred), hospice experience preferred, and a valid driver’s license. Join a supportive team with opportunities for professional growth.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must have a valid driver’s license and reliable vehicle.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patients’ wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will:

Qualifications
