Job summary

Hospice of North Alabama is seeking a dedicated RN Case Manager to join our Cullman, AL team. This role focuses on comprehensive patient assessments, care planning aligned with patient wishes, and delivering direct nursing care in a home-based hospice setting.

The ideal candidate will hold a nursing degree, possess active licensure, and have hospice experience. We offer a supportive environment with opportunities for professional growth and a strong emphasis on compassionate patient care.

Qualifications

• Graduate of an accredited nursing school with current state licensure.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patients wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications

• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
